When you first arrive at the agency, you will be greeted by our staff and asked to check in at the front window.
- Please have a valid picture ID, any Medicaid/Insurance card, and list of any medications.
- For ADSAP/DUI referral: Recent 10-year driving record; valid picture ID, and $250 fee
- Parents/Guardians will need to accompany adolescents to during the initial intake appointment.
You will then be given a client handbook while completing intake paperwork with the Admin. staff. This allows us to obtain basic information from you and orient you to our agency. After completing the intake process, a counselor will bring you to his/her office to begin the bio-psychosocial assessment.
During the assessment, you will be asked a series of questions that allows the counselor the opportunity to get to know you. The counselor will take a Person-Centered approach to meet your quality of care. Following the assessment, you will develop an individualized plan of care to meet your goals of treatment.
The assessment process may take up 2 hours to complete.
